For an evaluation or assessment you can expect to complete an intake session, testing sessions, and a feedback session. The intake session will consist of reviewing and signing the clinical forms once you have read them. Forms include the following: Consent Form, Child Assent Form (if applicable), Financial Policy, Privacy Practices Form, Disclosure Form, and Consent to Release/Obtain Information (if applicable). For the remainder of the intake session a semi-structured intake will be completed assessing your current concerns and purpose for the evaluation. Additional history information will be discussed, for example, past counseling experiences, current medications, family background, and educational history.
For child clients information will be discussed regarding developmental background, educational history, previous testing, behavior concerns, and social concerns. The testing sessions will take anywhere between 2 hours to 8 hours depending on the type of evaluation required. In addition to the psychological/cognitive evaluation completed, a school visit can be conducted when necessary to assess your child's behavior and performance in the classroom environment.
Upon completion of the evaluation a detailed report will be completed and discussed with you approximately one week following the last testing day. During this feedback session the findings and recommendations will be discussed with you in detail. Additionally, you will be provided the original report and two copies of the report, if needed. Your therapist can also attend your child's Individual Education Plan (IEP) meeting when necessary.
